What Actually Causes a Mini Stroke (TIA)?

It’s 1am, phone light in your face, and someone’s hand went numb for four minutes before it came back to normal.
If that happened in the last few hours, or if anything like it (sudden numbness, slurred speech, a drooping face, trouble seeing or walking) happens again, even for a minute, stop reading and call emergency services or get to an ER now. Don’t wait to see if it passes on its own again.
Remember FAST: Face drooping, Arm weakness, Speech difficulty, Time to call emergency services. A TIA is sometimes called a “mini stroke,” but it’s treated as a full emergency in the moment, because paramedics and ER staff can’t tell a TIA from a stroke just from the symptoms alone.
If you’re reading this after you’ve already been seen, an ER doctor or urgent care told you “it looked like a TIA,” and you’re trying to understand what actually happened and what to do next, here’s the plain answer.
Here’s the plain-language version: a transient ischemic attack happens when something, usually a clot or a small piece of debris from a blood vessel, briefly blocks blood flow to part of your brain. “Briefly” is the key word. In a full stroke, the blockage sticks around long enough to damage brain tissue permanently. In a TIA, blood flow comes back, usually within minutes, sometimes up to an hour, and the symptoms fade with it.

A handful of specific mechanisms can cause a TIA, and figuring out which one applies to you is exactly what your doctor is doing with all those tests.
What’s Happening Inside Your Blood Vessels?
The most common story behind a TIA starts years before it happens, quietly, inside your artery walls.
It’s called atherosclerosis: your arteries slowly narrowing as fatty deposits, cholesterol, and other debris build up along the inner walls and harden into plaque. Picture a pipe that’s been collecting mineral buildup for a decade. Blood still flows, mostly fine, until something changes.
Sometimes a piece of that plaque, or a small clot that forms on its rough surface, breaks loose. It travels downstream until it hits a vessel too narrow to let it through, and for a few minutes, that clog cuts off blood supply to whatever patch of brain tissue depended on it. Then, often, the clot breaks up or shifts, and flow resumes. That’s the TIA, start to finish.
We bring this up first because atherosclerosis sits underneath a large share of TIAs, and it’s connected to nearly every modifiable risk factor further down this page. High blood pressure damages artery walls and makes them more likely to collect plaque. High cholesterol supplies the raw material. Smoking speeds up the whole process. High blood sugar from diabetes quietly damages vessel linings over time.
One process, several different levers feeding it, which is actually good news: move one lever and you’re working on all of it at once.
Can an Irregular Heartbeat Cause a TIA?
Yes, and this is the one that surprises people most. A fair number of TIAs start in the heart, well away from the brain itself.
The main culprit is atrial fibrillation, an irregular and often rapid heart rhythm where your heart’s upper chambers quiver instead of beating in their normal, coordinated pattern. Think of it like a drummer losing the beat: the chambers flutter instead of squeezing in sync, and when that happens, blood can pool in one of them, most often a little pocket called the left atrial appendage, instead of moving through smoothly. Pooled blood has time to clot.
If a clot forms there and a piece breaks free, it travels through the bloodstream and can lodge in an artery feeding the brain, the same basic mechanism as the plaque scenario above, just with a different starting point.
Atrial fibrillation can be completely silent, and plenty of people have it without ever feeling the fluttering or skipped beats you’d expect. That’s one reason a heart rhythm check, often a period of monitoring lasting anywhere from a day to a few weeks, is a standard part of working up a TIA.
That monitoring usually means wearing a small patch or clip-on monitor under your shirt for a few weeks, an easy routine that fits under your clothes and barely interrupts your day. Still, it’s worth asking your doctor’s office what it costs before you agree to it, since coverage and co-pays for cardiac monitors vary a lot by insurance plan. If AFib turns out to be the source, your doctor’s plan for you will look different than it would for narrowed arteries, and that’s a conversation to have directly with them.
Is Narrowing in the Neck Arteries a Cause?
It can be, and it’s specific enough to get its own diagnostic workup.
The carotid arteries run up either side of your neck and are the main highway supplying blood to your brain. When plaque builds up inside them, it’s called carotid artery disease, essentially the same atherosclerosis process described above, just concentrated somewhere the consequences are especially direct.
A narrowed carotid artery can throw off small clots or bits of plaque debris that travel a short, fast distance straight into the brain’s blood supply. Because the carotid arteries sit close to the surface of the neck, doctors can check them fairly easily, usually with an ultrasound. Imaging of the neck arteries is a routine part of almost every TIA workup, whether or not carotid disease ends up being the cause.
Carotid narrowing doesn’t always announce itself with symptoms before a TIA happens, which is part of why routine cardiovascular checkups matter even when you feel completely fine. It’s also a quick, painless scan, more like a blood pressure check than anything invasive, so there’s no reason to dread it if your doctor orders one.
Which Risk Factors Are Within Your Control?
This is the part worth sitting with, because it’s the part you can actually do something about.
Several of the biggest contributors to TIA risk respond to small, specific changes. Small, consistent moves in the same direction are what actually work here.
- High blood pressure. Consistently elevated blood pressure is widely considered one of the biggest modifiable risk factors for stroke, and the same vessel damage drives TIA risk. If you haven’t had it checked in the last year, a home cuff runs under $30 and takes about two minutes to use.
- Smoking. It damages blood vessel linings, raises the tendency of blood to clot, and speeds up atherosclerosis. If you smoke, your doctor can talk through options like a patch, gum, or medication that make quitting easier, worth bringing up at your next visit. The vessel benefit tends to show up within a few months of quitting.
- Blood sugar and diabetes. Chronically high blood sugar damages blood vessels from the inside, including the small ones. If it’s been a while since you checked, your doctor can run an A1C test at your next visit. It’s one blood draw and gives you an actual number to work from.
- High cholesterol. Specifically LDL, the kind that contributes directly to plaque. A lipid panel is one more blood draw, and it’s one of the more straightforward numbers to track and manage with your doctor over time.
- Inactivity. You don’t need a gym membership. A 20-minute walk most days moves blood pressure, cholesterol, and blood sugar all at once, which is part of why it shows up on nearly every cardiovascular risk list.
- Carrying excess weight, particularly around the middle. Waist size tracks more closely with blood pressure and blood sugar risk than the number on the scale does, so if you’re going to measure something, measure that instead.
- Heavy alcohol use. It’s linked to high blood pressure and irregular heart rhythms, both of which loop right back to the causes above. Cutting one drink a week counts on its own, no need to wait until you’re ready for bigger changes.
You don’t have to overhaul your life by Tuesday. Moving any one of these levers, even partway, changes the odds in your favor. If you’ve had a TIA, your doctor will likely check most of these numbers directly rather than guess, so you’ll know soon enough which ones actually need your attention.
Which Risk Factors Aren’t Within Your Control?
And then there’s the list you can’t do anything about, which deserves to be named plainly rather than danced around.
- Age. Risk rises as you get older, particularly past 55, simply because blood vessels have had more time to accumulate wear.
- Family history. Having a parent or sibling who’s had a stroke or TIA raises your own risk, likely a mix of shared genetics and shared habits.
- A previous TIA or stroke. Having had one before is one of the strongest predictors of having another, which is exactly why a TIA gets taken so seriously even though the symptoms went away on their own.
- Sex and race. Risk differs somewhat by both. Black Americans face notably higher stroke risk than white Americans. Higher rates of high blood pressure and diabetes play a role, and so do broader access and disparity issues in healthcare. TIA tracks the same pattern, for the same underlying reasons.
There’s no fixing any of this, and that’s fine. Knowing it tells you how hard to work the list you can move. If you’re 68 with a parent who had a stroke, your blood pressure number carries more weight than it would for someone with neither of those in the picture.
Do Other Health Conditions Raise the Risk?
Beyond the major mechanisms above, a handful of other conditions show up often enough in TIA workups to be worth knowing.
Sleep apnea, where breathing repeatedly stops and starts during sleep, is linked to higher blood pressure, irregular heart rhythms, and inflammation in blood vessels, all of which feed into TIA risk. It’s easy to miss because loud snoring and daytime exhaustion don’t sound like a heart or vessel issue, but doctors are increasingly asking about sleep as part of a full risk picture. If you snore heavily or wake up exhausted no matter how long you slept, mention it at your next appointment. It’s a five-minute conversation that can lead to a simple at-home sleep test.
Certain clotting disorders, some inherited and some acquired, make blood more prone to forming clots than it should. Some heart valve conditions, including valve damage from past infections or degeneration, can create the same kind of turbulent blood flow that atrial fibrillation does, giving clots a place to form. These show up less often than atherosclerosis or AFib, but they’re part of why your doctor might ask questions that feel like they’ve got nothing to do with your blood pressure, family history of clots, past miscarriages, that kind of thing, and no, they haven’t lost the thread. If your workup includes a clotting panel or an echocardiogram, that’s why.
Why Does Figuring Out the Cause Matter?
Because the cause is what your doctor uses to decide what happens next, and that plan genuinely differs depending on the answer.
This is exactly why doctors move fast after a TIA: heart monitoring, carotid imaging, blood work, sometimes brain imaging, often within days rather than waiting to see if anything happens again. A TIA is treated as a warning sign because it usually points at a specific, often fixable mechanism, and finding that mechanism is the whole point of the workup.

The one question worth bringing to that follow-up appointment is simple: what did the workup find, and what does that mean for me specifically.
